In June of 2013 TenderCare Clinic earned Level 3 NCQA Patient Centered Medical Home (PCMH) Recognition under the 2011 standards. This is the highest level a practice can achieve. 

The patient-centered medical home model encompasses five key elements:

 *   Patient-centered: It is relationship-based, focusing on the whole person. Your provider will respect your unique needs, culture, values, and preferences. We recognize that you and your family are important members of your health care team, and will help you learn to manage your own care.

  *   Comprehensive care: We are committed to meeting all your physical and behavioral health needs including prevention and wellness, acute care, and chronic care. Your care team works together to ensure the best care possible. This team might include physicians, advanced practice nurses, physician assistants, nurses, pharmacists, nutritionists, social workers, educators, and care coordinators.

  *   Coordinated care: Your team will coordinate your care across all elements of the broader health care system, including specialty care, hospitals, home health care, and community services and supports.

  *   Improved access to your Primary Care Team: Our goal is to be as responsive to your medical needs as possible.

  *   Continuous Improvement:  We are committed to ongoing measurement of our progress in improved treatment outcomes and excellent service to you, our patients.

The National Committee for Quality Assurance is a private, 501(c)(3) not-for-profit organization dedicated to improving health care quality. Since its founding in 1990, NCQA has been a central figure in driving improvement throughout the health care system, helping to elevate the issue of health care quality to the top of the national agenda.

 


What is a Patient Centered Medical Home?

  • The patient centered medical home model is a way of running a medical practice to help better coordinate care and communicate with patients. It is an extension of the traditional role of primary care.

  • A medical home actively seeks to coordinate care across multiple settings. This includes striving for the best communication possible with specialists and hospitals.

  • A medical home makes clear to patients where they should go for medical assistance at any time. This includes posting clear office hours, where to seek care after hours, and how to communicate with the office during and after business hours.

  • A medical home gathers and incorporates your medical information in one place. This includes your current medications, notes from specialists, medical history, health status, test results, hospital visit information, etc.

  • A medical home provides evidence based care whenever possible. This means that medical decisions are based on studies that prove that treatments are effective, rather than doctors just having to use their best judgment. We also strive to empower patients to care for themselves as much as possible by facilitating patient education and directing patients toward good medical information.

This means that our center as your Medical Home is a central resource for all your health care needs. You are welcome and invited to participate in all aspects of your care.

As Your Medical Home We Will:

  • Help you find needed information and resources such as information about specialists, health conditions and latest treatments, home care, equipment and vendors, supports and respite services for your family, other key local services.

  • Take care of you when you are sick and well and help you to stay well.

  • Help you plan for your care and/or set goals for your care, now and in the future.

  • Talk with you about any testing or treatment that you may need.

  • Work with you and other care providers to coordinate care.

You Can Make the Most of our Services if You:

  • Are prepared to share any updates since the last visit (i.e. new over-the-counter medications, any ER visits, etc.)

  • Write down and bring in your main questions and concerns.

  • Bring your current medication bottles with you to your appointment.

  • Bring a list of any other providers you see with their contact information to your appointment.

  • Ask for help in creating and sharing a complete record of your care.
